Your transition to adult support starts here

Moving into your own home is exciting, but it can also feel a bit scary.

We will be with you all the way, helping you make the most of it.

Perhaps you’ve been at a residential college and are now transitioning to adult support, or maybe you’ve lived with your family for many years.

Either way we will work with you, your family, the professionals, and your support circle to plan your move carefully.

We take it step by step. We’ll help you understand your options, your rights, and your responsibilities.

Our goal is to help you become more independent. For example, at High Greave, our specialist transitions home in Sheffield, young adults are getting their first jobs, going on dates, cooking for themselves, taking medication independently, travelling alone, and more.
