The shape of your life

You’ll make choices that matter

We’ll plan together for work, hobbies, relationships, health, home and housemates.

In 2024 we helped 151 people take control of their life through Individual Service Funds (ISFs.)

Your Person-Centred Support

You are at the centre of your support – we call this person-centred, active support.

Personal and meaningful support

Our support plans are personal, creative and collaborative – reflecting your character and expressing your gifts, ambitions and needs in a way that means something to you.

We ask person-centred questions. These lead to great person-centred conversations (and real understanding.) Ultimately you’ll experience great person-centred decisions and outcomes.

Support Planning

You’ll get person-centred, active support that increases the range of choices you can make, and so the amount of control you have over your life.

We start by understanding who you really are.

Your support plan helps us find out what is important to you, and for you, and is created with you and the people that matter to you.

The support plan is the start of our relationship with you. It is an active document that you own. It guides us in supporting you to achieve the things that matter to you and to stay connected to the things you love.

This sets you up for a full and happy life.

Annual Reviews

Every year, we celebrate your achievements and your life through a Person-Centred Review.

This reminds us what matters to you, helps you to see what’s possible, identifies challenges and agrees how we can best support you to live the life you choose.

What are Person-Centred thinking tools?

Person-centred thinking tools are a collection of easy-to-use templates designed to provide structure to conversations. We use these to capture information that contributes to person-centred planning, as well as enhancing understanding, communication, and relationships.

Some of these tools form part of your support plan. Person-centred tools help us to get to know people well, and iPlanit is where we record and share this information. Families can access this information, together with daily records, photos and your calendar through our family portal.

What does a Person-Centred Review look like?

It will be different for every person, that’s what makes it person centred. As much as possible, it is led by you – it is about you, it is for you, and it is done with you.

You can invite anyone important to you to contribute beforehand and on the day.

Person Centred Reviews are tailored to your gifts and your preferred means of communication. They’re creative, fun, engaging and celebratory. They usually take about two hours and happen at the most convenient day and time for you.

Our ‘Activate’ model

Person centred active support is about making every moment in a person’s life count. We have the privilege of enabling you to live the life you choose, fully and happily.

Every moment in your day has potential and our advanced model of support, Activate, encourages us to see the potential in every moment. Activate is the way we set goals with you in different parts of your life, and then work out steps to help you achieve those goals.

We use the Activate method and the iPlanit app to organise our person-centred support in Dimensions.

Video support plans

Some of the people we support have a video support plan that enhances their person-centred support plan (without replacing it.)

Video helps us demonstrate aspects of a person’s support and personality that are hard to capture in words.  It can help colleagues support a person consistently in the same way, and so reduce anxiety.

Video is particularly good at illustrating a person’s preferences, routines and how they respond to the right support. This can increase the level of control a person has over his or her life.
