Here’s how you can enter yourself, or nominate somebody else, for the Learning Disability and Autism Leaders’ List 2019.

You can:

Anybody with a learning disability and/or autism can enter the Leaders’ List 2019. And, anybody can nominate somebody who has a learning disability and/or autism. Continue reading to find out more.

How can I share my story?

Write your entry, record a video or complete our easy read form to enter.

You can tell us your story by:

  • Answering our questions using the online form

OR

  • Recording your answers in a short video

OR

  • Printing off and completing the Easy Read form

What will I be asked?

You will be asked for some personal details and to choose a category.

To enter the Leaders’ List we need to know your story. But, we also need to know some more information about you.

First, we will ask you for some personal details

Don’t worry, we won’t share this information with anybody else without asking you first. This information will help us understand you better and get in touch with you.

If you are entering yourself, we will ask for your:

  • Name
  • Age
  • Postcode of your home address
  • Any organisations you work for or are supported by
  • Details about your learning disability and/or autism
  • Contact details

If you are nominating somebody else, we will ask for the above details about the person you are nominating, as well as your name and contact details so we can get in touch with you.

Second, we will ask you to choose a category for your entry

There are six categories you can choose from:

  • Local communities
    This is for people who are making a difference or getting involved in their local community.
  • Advocacy, policy and the media
    This is for people who are trying to make their communities or the country a better place with more people included and everyone respected.
  • Work and education
    This is for people who are in paid work or learning new things.
  • Sports, arts and entertainment
    This is for people who are doing creative and athletic things.
  • Groups and teams
    This is for groups and teams who are working together to achieve things.
  • Profound and multiple disabilities
    This is for people who have profound and multiple learning disabilities and are achieving things.

If you’re not sure what category to select don’t worry, we will make sure your story is in the best category for you.

Third, we will ask you to answer these questions

Whether you choose to write your answers onto our form or record your answers in a video, we would like you to answer these questions.

Please try to keep your answers short and easy to understand. This is because the majority of our judges have a learning disability and/or autism:

  1. Please tell us why you are entering the Learning Disability and Autism Leaders’ List
  2. Please tell us why these achievements are important to you
  3. Please tell us what challenges you have faced and what you have learned
  4. Please tell us what other people have said about the great things you have done
  5. Please tell us anything else you would like us to know

Take some time to think about what you would like to say.

Please keep written answers to 3 medium size paragraphs or less and videos under 10 minutes long.

Finally, we will ask you to upload some photos

These are the photographs we might use if you are chosen as one of our Leaders so please choose some really nice photos of you. Don’t worry, we won’t use them for anything without asking you first.

You can upload up to 3 photos in JPEG or PNG format.

How to submit your entry

Submit your entry.

You can submit your entry using our online form or sending us the completed printed form.

How to submit a video entry

To submit a video entry, you must first upload it online. You can do this using YouTube or Vimeo.

If you don’t want anyone else to see your video, please upload it as “unlisted” or password protect it (remember to tell us the password).

You can share the link to your video using the online form.

How to submit a written entry

If you are submitting a written entry using our online form, you can write your story in the boxes on the online form.

If you have downloaded the Easy Read form you can upload a scanned copy to the online form.

The Leaders’ List is a celebration of people’s stories. If your entry is selected as one of the winning stories, you (or the person you nominated) will need to complete a consent form. If full consent isn’t provided, we will have to approach another person.
