What we do
We support young people aged 14-25 in transition and their families. We work closely with them to develop individualised support packages. These may include supporting the young person to access leisure opportunities or education or move into employment.
We also provide support as they look at the options open to them regarding housing and the alternatives to traditional residential care.
Who we work with
We work with a number of local authorities and with the Transition Information Network (TIN) – a group of organisations working to improve the lives and experiences of disabled young people's transition to adulthood.
