Transition

Kelly, who is supported by our transitions serviceThere comes a stage in every young person's life when they face many options and changes. Leaving school, finding a job, choosing a training course and moving out of the family home can be just some of the choices that arise.

This is just the same for those with learning difficulties and autism. However, a greater level of support and planning is needed to ensure their future path runs smoothly.

And that's where we can help…. We provide the specialist support needed by young people with learning difficulties and autism as they move into adulthood; a phase often referred to as transition.

Our Transition Service supports those aged between 14 and 25, helping them to get the most out of life and giving them the chance to follow the path that they want.
