Person Centred Planning
What is person centred planning?
Person centred planning is a process of life planning for people. Person centre planning ensures that the person is central to all planning and decisions. The plan is owned and controlled by the person [and sometimes their circle of support]. Each plan is unique to the person.
Is this a new idea?
No, person centred planning has been around for a number of years. It replaces the more traditional styles of assessment and planning which were based more on a medical model of support.
Is there a cost involved?
Thinking in different ways does not necessarily cost money. Sometimes people's dreams and aspirations may have a cost attached but not necessarily the planning process.
Dimensions has skilled facilitators who can assist with the planning process.
Some agencies may attribute costs to person centred planning for the time element involved and depending on the support package.
Who is involved?
Whoever the person wants to be involved. Often this will be those who love and care about the person but equally it may be other people that the person wants to be included.
How long does it take?
Each person is different. There are no hard and fast rules about planning and timescales. It takes as long as is needed for the person.
I have a support plan is this the same thing?
We like to describe support plans as 'what you need today' and person centred plans as 'what you need tomorrow', plans for the future. Having said that though, all plans should be person centred and focus on the person.
Who owns the plan?
Each plan is owned by the person.
What happens when the plan is finished?
Planning is an ongoing process, life plans are never finished. We might achieve some of our goals and then we look for more. Everyone has dreams and goals, some of which we will achieve and some of which we wont, but we might get somewhere nearer to them and we might also change our minds about what we want!
How can I get a person centred plan?
There are a number of people who can help you to get a plan including your support staff, your care manager or your family and friends.
