What we do
We support young people aged 14-25 in transition and their families to develop individualised support packages. These may include supporting the young person to access to leisure and community participation, continued learning and employment.
We also provide support to source housing options and explore with young people (and their circle of support) alternatives to traditional residential care.
We can offer impartial advice and guidance from a team of regional transition leads.
We appreciate the anxiety and uncertainness that transition can cause and work tirelessly to create a seamless service which can change and develop as the young person reaches adulthood.
Who we work with
We work with a number of Local Authorities to determine their future strategic actions for transition work. We also work with the Transition Information Network (TIN), an alliance of organisations working to improve the lives and experiences of disabled young people's transition to adulthood.
Referrals
Many of the referrals we currently receive are from care managers supporting young people 18 or over coming back into county but we actively encourage work with younger people preparing to make the move from children's to adult services.
